What Are Clear Aligners?

Clear aligners are a set of custom-fabricated, transparent plastic trays — each tray is worn for 1–2 weeks and moves your teeth a fraction of a millimetre closer to the target position. A full course of treatment involves anywhere from 10 to 60+ trays depending on complexity. Unlike braces, aligners are removable — you take them out to eat and clean your teeth, then put them back. The trays are nearly invisible in day-to-day wear.

Traditional metal braces are effective but involve fixed brackets and wires that cannot be removed, make oral hygiene difficult, restrict food choices, and are visually prominent. Clear aligners address all of these limitations. They are particularly popular with working adults who want to correct their teeth without the social visibility of metal braces. The trade-off is that compliance is entirely self-managed — aligners only work if the patient wears them consistently.

Clear aligners are appropriate for mild to moderate crowding, spacing between teeth, mild rotations, and minor bite discrepancies. How aligners actually move teeth

Aligner tray

Made from medical-grade thermoplastic (typically SmartTrack for Invisalign, or similar proprietary materials for other brands). Each tray is trimmed at the gum line and exerts gentle, precise pressure on specific tooth surfaces.

Attachments (buttons)

Small tooth-coloured composite bumps bonded to specific teeth. They act as handles — the aligner grips them to apply forces that plain trays cannot (rotations, intrusion, extrusion). Not every case requires attachments, but moderate cases usually do.

IPR (Interproximal Reduction)

Slight controlled removal of enamel between crowded teeth to create space. Performed with a thin diamond strip. Typically 0.1–0.3mm per contact — within safe limits. A necessary part of many aligner treatment plans that is sometimes not explained to patients.

Refinements

Additional sets of trays ordered mid-treatment to correct teeth that haven't moved as planned. Almost all cases require some refinements — this is normal, not a failure. The number of refinement trays should be included in your treatment cost.

Retainers

Clear trays worn after treatment to hold teeth in their new position. Teeth have a strong memory — without retainers, they will move back toward their original position within months. Retainer wear is lifelong (typically nightly). This cost is often not included in the initial quote.

Clear Aligner Treatment: Step by Step

  1. Clinical assessment and case suitability

    The orthodontist examines your teeth, jaw, and bite to determine whether aligners are appropriate for your case. X-rays and photographs are taken. This is the most important step — and the one most often skipped by direct-to-consumer aligner brands who rely on at-home impressions without clinical oversight.

    Never start aligner treatment without an in-person clinical examination by a qualified orthodontist. At-home impression kits bypass this step, which can result in treatment plans that don't account for gum health, bone levels, or bite problems.

  2. Digital scan or impressions

    A 3D digital scan of your teeth is taken with an intraoral scanner (preferred) or traditional putty impressions are made. This data is used to fabricate the aligner trays. The scan is also used to create a digital simulation (ClinCheck for Invisalign, or similar) showing your projected tooth movement tray by tray.

  3. Treatment plan review

    Your orthodontist reviews the digital simulation and makes modifications to the automated tooth movement plan. This step is where clinical expertise matters most — the software-generated plan often needs adjustment for biological limits of tooth movement, anchorage, and bite coordination.

    Ask to see your ClinCheck or equivalent simulation before trays are ordered. If the clinic cannot show you a movement simulation, ask why.

  4. Attachment bonding and IPR

    At the start of active treatment, composite attachments are bonded to specified teeth. Any required IPR is performed at this stage or progressively as treatment advances. Local anaesthesia is not needed for either procedure.

  5. Active tray wear

    You change trays every 1–2 weeks (as prescribed). Each tray must be worn 20–22 hours a day — removed only for eating, drinking anything other than water, and tooth brushing. Monitoring appointments are every 6–10 weeks to check progress and identify any teeth not tracking correctly.

    Do not skip monitoring appointments. Teeth that are not tracking — not moving as planned — will not correct themselves by continuing to the next tray. Catching this early prevents the need for extensive refinements later.

  6. Refinements

    Once the initial set of trays is complete, your orthodontist assesses the result against the original plan. Almost all moderate and complex cases require additional refinement trays to complete tooth movements that didn't fully express. A new scan is taken and additional trays are fabricated.

    Ask before treatment starts: are refinements included in the quoted price, and how many rounds are covered? A quote that excludes refinements is misleading.

  7. Retainer fitting

    Once treatment is complete and the result is confirmed, permanent retainers and/or removable clear retainers are fitted. This is the final and most critically underemphasised step in aligner treatment. Without retainers, teeth relapse. Most patients need to wear removable retainers every night indefinitely.

    Retainers are not optional and not a one-time purchase. They wear out and need replacing every 1–2 years. Factor this into your total treatment cost.

What affects the price?

Case complexity

Simple cases (minor crowding, small gaps) require fewer trays and less clinical time. Complex cases (multiple rotations, bite issues) require more trays, more refinements, and more monitoring — all of which add to cost.

Monitoring frequency

In-person monitoring every 6–10 weeks is standard. Some remote-monitoring plans require only quarterly visits, reducing travel — but increase the risk of missed tracking problems.

Each new tray causes mild pressure and aching for the first 24–48 hours as the teeth begin to move. This is normal and typically manageable with ibuprofen if needed. Most patients describe it as discomfort rather than pain, and it diminishes significantly after the first few trays. If a tray causes sharp pain or feels drastically different from the previous one, contact your orthodontist.
What is the difference between Invisalign and Indian brands like toothsi or makeO?
Invisalign trays are made in the US from a patented material (SmartTrack) with a 35-year clinical track record. Indian brands manufacture locally at lower cost — material quality is adequate but the evidence base is smaller.
